This second edition of Netter’s Cardiology benefited enormously from the hard work and talent of many dedicated individuals. First, we thank the contributing authors. All are current or former faculty members at the University of North Carolina School of Medicine, Chapel Hill, or have close ties to the institution. Without their intellect, dedication, and drive for excellence, Netter’s Cardiology, 2nd edition, could not have been published. We had a solid foundation on which to build the second edition, thanks to the hard work of the first-edition contributing authors, many of whom we were fortunate to have continue on to this edition. We are also grateful for the invaluable editorial contribution that Dr. E. Magnus Ohman made to the first edition.

This book is the culmination of many hours of hard but very satisfying work. Management of Wilson Disease

The cornerstone of achieving best care and maximizing outcome for disease management is the partnership between health care provider and patient. This “Handbook” was therefore targeted to primary physicians to help them understand more about the underlying physiology, diagnosis, and treatment of the rare disorder, Wilson disease. We have focused the subjects included on what we consider relevant care issues for patients. Patients may also benefit from reading this book and discussing specific subject matter with their caregivers. Whether read by the primary care or other treating health care provider or patient, our aim is to stimulate conversation between patient and their caregivers in order for them to achieve the individualization of care they need, recognizing that many patients have different expression of their disease and differing degrees of disability.

HAEMOSTASIS IN SURGERY

It is significant that the first medical description of the clinical and genetic features of haemophilia was by the ‘‘father of surgery’’ Abu al-Qasim Khalaf bin ‘Abbas el-Zahrawi (940?--1031 C.E.), so called for his extensive original descriptions of operative techniques and instrumentation. Characteristically he also described a method for stopping bleeding in haemophilia by local pressure and cauterisation, which he had witnessed.

Social phobia and disruptive social anxiety are features of the lives of many thousands of people. But exactly what is social phobia? What causes it? What is its nature and what kinds of treatments can improve it? Using key concepts and methods and a substantive body of research, this book aims to answer these questions and clarify social phobia by means of critical discussions and examination of evidence. It takes a skeptical stance towards the received view of social phobia as a species of disease caused by a deficient inner mechanism and considers and alternative construal of social phobia as a purposeful interpersonal pattern of self-overprotection from social threats. The possibility that social phobia might not actually exist in nature is also considered.
